Have all employees been provided with a standard lock and personalized key? Are employees required to ensure they keep their personal key with them at all times? Is all equipment designed to facilitate lock out and tag out? Are all appropriate circuits properly identified? Have all appropriate employees received training on standard lock out & tag out procedures? Has this training been conducted by a qualified person with a background in electrical theory? Are there checks performed on the effectiveness of the lock out and tag out practices? Are deviations from the lock out & tag out standards corrected? Is recognition given when the lock out & tag out standard is working as required? Standards Protection Code Is an up to date standard protection code in place? Has the standard protection code been developed for the unique electrical risks at the worksite? Are sufficient copies of the standard protection code available? Do employees who require a copy of the standard protection code have access to the code? Is the code updated as standards or equipment changes? Has training been conducted on the standard protection code? Have all required employees received training as required? Has the training been evaluated for effectiveness? Is the training updated as required? Is there an annual training plan for the standard protection code?
